Lead Paint Forms
Lead Paint Forms are available at the Vermont Department of Health website.
For those who use Trueforms, you can access the new lead paint forms and handouts by updating your Trueforms. If you use automatic updates, simply login into Trueforms and click update. If you update manually, login to Trueforms, click on Tools and click update.

Paper Forms
You can pre-order your paper forms by downloading and faxing a forms worksheet to NVBR. We will prepare your forms and have them ready for you to pick up at your convenience. Following is a list of forms and prices available from NVBR.
